Sports Reminder

Dear Secondary Parents,

School is just around the corner and it’s time to start thinking about fall activities (volleyball and cross country) at Logos. On this site, under athletics, are three forms, some or all of which will need to be completed before any student may participate in activities here at the school.

1. Waiver and Release of Liability form. Every secondary student (7-12 grade) participating in any activity (athletic or non-athletic) must have one of these on file with the activities director before the first day of practice. If you have more than one secondary student participating, a single form per family is fine.

2. Interim Questionnaire (IHSAA) form. Every high school student (9-12 grade) participating in an athletic activity (volleyball, cross country, basketball, track, or lacrosse) must have one of these on file with the activities director before the first day of practice.

3. Health Examination and Consent (physical) form. Every 9th and 11th grade student participating in an athletic activity must have a physical performed and this form completed by the physician before the first day of practice. The parent completes the front side of the form and the physician the back side.

Volleyball: The high school teams (varsity and junior varsity) start practice Friday, August 15 at 5:45am in the gym. Before athletes may participate they need to bring a completed “Interim Questionnaire” form, a signed “Waiver and Release of Liability” form, and the $55 participation fee with them to the first practice. In addition, 9th and 11th graders must have a health examination from a doctor (see above). Students will not be allowed to practice without the proper forms and fees.

The junior high team doesn’t start until the second day of school, August 26. Practice will be after school. They should have the “Waiver and Release of Liability” form and the participation fee of $30 turned in by then.

Cross Country: Cross Country (junior high and high school) also starts on August 15th at 7:30am (meet at the Moscow JH tennis courts). All the form and fee requirements for volleyball are the same for high school cross country participants. Junior high runners need only complete the waiver form and bring the $30 participation fee. Be sure to have your children bring everything with them to the first practice so that they will be able to practice with the team. Also, Coach Ryan would like Cross Country runners to begin working out on their own. The kids should be up to 3-4 miles (JH – 2-3 miles) 3 times a week and 6-7 miles (4-5 for JH) once a week by the time practice starts.
