Introduction to JH Activities

Forms

All 6th grade students who will be participating in a sport must have a physical on file. Physicals are required in grades 6, 9 and 11. All other years, a signed Interim Questionnaire(IQ) form is needed. To submit physicals and IQ forms you must create a DragonFly account for yourself and your child. Instructions for creating the account and uploading documents can be found on the Forms and Fees Page. Other forms and waivers can also be found on the Forms and Fees Page.

Fees

The fees for each sport can be found on the Activity Fees Page. Fees will be billed through the FACTS system unless alternate arrangements are made with the Activities Department.
