Story Family Medicine will be offering physicals for Logos Athletes. The physical can be scheduled for June 3 or July 22 by contacting Story Family Medicine directly. The cost of the physical is on a donation basis. All funds raised go to support the Logos training department. You do not have to be a member of Story Family Medicine.

Sports Physicals Requirements: All students entering grades 6, 9, and 11 must submit a physical dated after May 1, 2023, to be eligible to participate in Logos Athletics.
