Sports Physical Form
Every 9th and 11th grade student participating in an athletic activity must have a physical performed and this form completed by the physician before the first day of practice. The parent completes the front side of the form and the physician the back side.

Interim Questionnaire Form
Every high school student (9-12 grade) participating in an athletic activity (volleyball, cross country, basketball, track, or lacrosse) must have one of these on file with the activities director before the first day of practice.

Lacrosse: NILL Liability Waiver

These two lacrosse forms must be filled out and turned in before the first day of practice (NILL is the North Idaho Lacrosse League).
